Case Management Professional

Project Self-Sufficiency is a non-profit organization located in semi-rural northwestern New Jersey that has been assisting low-income families along the path to economic self-sufficiency for over 35 years. Our services include career guidance and assessment, computer classes, GED instruction, parenting workshops, childcare, legal assistance and education, help with emergency basic needs, health education, support groups, life skills classes, family activities, and home visitation. We have an exciting opportunity for a dedicated professional to join our team as a Housing First Specialist.
Job Description:
This role requires a compassionate and organized individual who can provide comprehensive case management services to individuals and families experiencing homelessness. As a case manager, you will work with vulnerable populations, assessing their housing needs, developing personalized plans, and linking them to relevant resources. This includes facilitating application processes for housing assistance programs, coordinating with landlords, and maintaining regular contact with clients through home visits and other communication methods. You will also advocate for clients' needs and rights within the community and with service providers. The ideal candidate has a Bachelor's degree in social work or a related field and minimum 2 years of experience working with similar populations.
Responsibilities:
- Provide comprehensive case management services to individuals and families experiencing homelessness.
- Assess clients' housing needs and develop personalized plans.
- Link clients to relevant resources, including medical, mental health, and social services.
- Maintain regular contact with clients through home visits and other communication methods.
- Advocate for clients' needs and rights within the community and with service providers.
Requirements:
- Bachelor's degree in social work or a related field (or equivalent experience).
- Minimum 2 years of experience working with vulnerable populations.
- Strong understanding of the Housing First model and principles.
- Excellent communication, organizational, and problem-solving skills.
- Valid driver's license and reliable transportation.
Salary Range: $60,000 - $70,000 per year, depending on previous non-profit management experience.
