Role Summary:

The Care Manager I is responsible for overseeing and coordinating the healthcare services for members of Capital Health Plan across various settings, including hospitals, skilled nursing facilities, and home health environments.

This role involves:

  • Assessing and ensuring that patients receive the appropriate level of care.
  • Facilitating discharge planning and coordinating follow-up medical services for patients under care management.
  • Connecting patients and their families with resources for services that may not be covered by insurance.
Qualifications:

Applicants must hold a valid medical professional license in the State of Florida. Proficiency in MS Office software is also required.
