Job Title: Registered Nurse

We are seeking a compassionate and skilled Registered Nurse to join our team at Vitas Healthcare. As a Registered Nurse, you will play a vital role in providing high-quality patient care and support to individuals and their families facing terminal illness and bereavement.

Job Summary:

The Registered Nurse will be responsible for coordinating patient care, ensuring continuous assessment of each patient's needs, and implementing the interdisciplinary plan of care. You will work closely with the interdisciplinary team to provide comprehensive care, including physical, medical, psychosocial, emotional, and spiritual support.

Key Responsibilities:
  • Coordinate patient care and ensure continuous assessment of each patient's needs
  • Implement the interdisciplinary plan of care and provide comprehensive care to patients and their families
  • Work closely with the interdisciplinary team to provide physical, medical, psychosocial, emotional, and spiritual support
  • Provide education and training to patients, families, caregivers, and referral sources in the care of the patient, including pain management
  • Participate in patient care meetings and weekly team meetings
  • Provide effective communication and shift reports through written, verbal, reporting, and body language
  • Participate in bereavement support and provide visits/shifts in program coverage area
  • Work with the IDG to develop, coordinate, and follow the patient's individualized plan of care
  • Obtain physician orders when necessary and provide those services requiring substantial and specialized nursing skill
  • Evaluate the outcome of services and appropriately delegate nursing tasks to Hospice Aides/Homemakers, Volunteers, and Licensed Practical/Vocational Nurses
Requirements:
  • Currently licensed to practice nursing in the state where the Vitas program is located
  • A minimum of two years of nursing experience in hospice, home health, or community health in the last five years
  • Equivalent experience or licensure may be considered
Working Conditions:

The physical demands for this role include assisting in lifting and transferring patients, lifting objects in excess of 20 pounds, standing or walking in excess of one hour per day, talking and hearing, and specific vision abilities required for the role include close vision, distance vision, and ability to adjust focus.
