Job Description:

The Licensed Practical Nurse (LPN) at Altru Health is responsible for delivering high-quality patient care and collaborating with the healthcare team to ensure optimal patient outcomes. This role involves conducting assessments, developing individualized care plans, and implementing interventions to promote patient well-being.

Key Responsibilities:
  • Assist registered nurses and other licensed practitioners in identifying patient needs, prioritizing care, and setting goals based on patient characteristics.
  • Contribute to the evaluation of care plans by gathering data, observing patient responses, and communicating findings to the healthcare team.
  • Demonstrate competency in performing department-specific functions, adhering to standards of care, policies, and procedures.
  • Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan.
Requirements:
  • Certified Licensed Practical Nurse (LPN) licensure in Minnesota or eligibility for temporary licensure prior to start date.
  • Basic Life Support (BLS) certification within 90 days of start date.
  • Possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
Working Conditions:

This position requires frequent standing, walking, stooping, bending, reaching, handling, seeing, and hearing. The incumbent must be able to lift, carry, push, and pull objects weighing up to 40 pounds. The working environment is primarily indoors, with occasional exposure to extreme temperatures not applicable to this role.
