Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Altru Health. As an LPN, you will play a vital role in providing high-quality patient care and ensuring the well-being of our patients.

About the Role

This position requires a skilled and detail-oriented individual who can perform and document assessments accurately. You will be responsible for completing health histories, admissions processes, and implementing interventions or referrals as needed. Additionally, you will identify discharge planning and education needs, provide patient care, and perform personal cares.

Responsibilities

  • Perform and document assessments appropriate to licensure
  • Ensure health history and admission process is completed
  • Implement interventions or referrals as needed
  • Identify discharge planning and education needs
  • Provide patient care and perform personal cares
  • Assist registered nurses or other licensed practitioners in identifying patient needs and priorities of care
  • Contribute to the evaluation of the plan of care by gathering, observing, recording, and communicating patient responses to nursing interventions
  • Modify the plan of care in collaboration with licensed practitioners
  • Demonstrate competency and ability to carry out department-specific functions in accordance with department standards of care, policies, and procedures
  • Function as a member of the multidisciplinary team, contributing to the implementation of an integrated patient-centered care plan

Requirements

  • Licensed Practical Nurse (LPN) license in Minnesota
  • Ability to work effectively in a fast-paced environment
  • Strong communication and interpersonal skills
  • Ability to maintain confidentiality and handle sensitive information
