Job Description:
The PACE Clinical Care Coordinator plays a crucial role in ensuring the delivery of high-quality care to our patients and their families. This position requires a highly skilled and dedicated professional who can provide exceptional nursing care in various settings.

Main Responsibilities:
  • Nursing Visits: Conduct routine and emergency nursing visits to patients in their homes, hospitals, and nursing facilities.
  • Patient Education: Educate patients and their families on disease management, medication adherence, and healthy lifestyle choices.
  • Care Planning: Collaborate with patients, families, and healthcare professionals to create personalized care plans that meet the patient's needs.
  • Coordination of Care: Ensure seamless communication and coordination of care among healthcare professionals, patients, and families.

Requirements:
  • Education: Degree from an accredited school of nursing required; BSN preferred.
  • Experience: Minimum one year of general clinical nursing experience in a certified home health agency, acute care, medical surgical, or critical care setting.
  • Licensure: Current New York State license to practice as a Registered Nurse.

Estimated Salary: $108,000 - $118,000 per annum.
