Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team as a RN Case Manager at the Karen Ann Quinlan Memorial Foundation. As a RN Case Manager, you will play a critical role in providing high-quality patient care and coordinating services to ensure the best possible outcomes for our patients and their families.

Key Responsibilities
  • Assessment and Planning: Conduct thorough assessments of patients' physical and psychosocial needs, develop and implement individualized care plans, and coordinate services to meet those needs.
  • Communication and Collaboration: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and address any concerns or issues that may arise.
  • Medication Management: Administer medications as prescribed, monitor patients' responses, and document any changes or concerns.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient care, including assessments, interventions, and outcomes, and report any changes or concerns to the healthcare team.
  • Case Management: Manage patient caseloads, prioritize tasks, and ensure timely completion of care plans and documentation.
  • Education and Support: Provide education and support to patients and families on disease management, treatment options, and self-care strategies.
  • Team Collaboration: Collaborate with the interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise.
Requirements
  • Education: Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred.
  • Licensure: Licensed to practice as a Registered Nurse in the State of New Jersey; licensure in Pennsylvania if practicing in PA.
  • Experience: Minimum one-year adult, pediatric, or medical/surgical clinical experience.
  • Skills: Strong nursing assessment and documentation skills, clinical knowledge to support the dying process, ability to serve as a resource to patients and families during times of high stress and volatility, excellent interpersonal and communication skills, and proficiency in operating a computer, including Microsoft applications and an electronic medical record.
