Position Summary: We are currently looking for a dedicated Registered Nurse to join our team at Bristol Glen Senior Care Community. Our facility is designed to provide personalized care in a nurturing environment, ensuring the highest quality of life for our residents.
Key Responsibilities:
- Manage the admission, transfer, and discharge processes for residents.
- Conduct comprehensive nursing assessments to evaluate residents' health statuses.
- Develop and implement individualized care plans, participating in care plan meetings as required.
- Document nursing activities in the Electronic Health Record (EHR) in accordance with established standards.
- Contribute to the resident's assessment through MDS/RAP documentation.
- Complete incident reports for any occurrences during the shift.
- Identify and assess changes in residents' conditions, notifying physicians or licensed practitioners promptly.
- Accurately receive, transcribe, and execute physician orders.
- Collaborate with residents, healthcare partners, and family members as appropriate.
- Initiate referrals to community agencies and healthcare resources to ensure continuity of care.
- Administer medications and treatments as prescribed, maintaining accurate records in the EHR while adhering to medication administration protocols.
- Ensure accountability for narcotics and medication cart security during shifts.
- Participate in mandatory training sessions and complete annual educational requirements in a timely manner.
