Position Summary: We are currently looking for a dedicated Registered Nurse to join our team at Bristol Glen Senior Care Community. Our facility is designed to provide personalized care in a nurturing environment, ensuring the highest quality of life for our residents.

Key Responsibilities:

  • Manage the admission, transfer, and discharge processes for residents.
  • Conduct comprehensive nursing assessments to evaluate residents' health statuses.
  • Develop and implement individualized care plans, participating in care plan meetings as required.
  • Document nursing activities in the Electronic Health Record (EHR) in accordance with established standards.
  • Contribute to the resident's assessment through MDS/RAP documentation.
  • Complete incident reports for any occurrences during the shift.
  • Identify and assess changes in residents' conditions, notifying physicians or licensed practitioners promptly.
  • Accurately receive, transcribe, and execute physician orders.
  • Collaborate with residents, healthcare partners, and family members as appropriate.
  • Initiate referrals to community agencies and healthcare resources to ensure continuity of care.
  • Administer medications and treatments as prescribed, maintaining accurate records in the EHR while adhering to medication administration protocols.
  • Ensure accountability for narcotics and medication cart security during shifts.
  • Participate in mandatory training sessions and complete annual educational requirements in a timely manner.
