Position Overview:
The Clinical Nurse Supervisor plays a pivotal role in delivering nursing care and overseeing the quality of care provided in alignment with the clients' individualized care plans.
Qualifications:
To be considered for this position, candidates must meet the following criteria:
- Graduation from an accredited nursing program.
- Current licensure as a Registered Nurse in the relevant state.
- A minimum of one year of experience in a home health care environment or hospital.
- Familiarity with Medicare and Medicaid regulations.
- Proficient in organizational skills and professional nursing documentation.
- Strong time management abilities and understanding of management processes.
- Ability to enhance the quality of care through effective communication with nursing leadership and team members.
Key Responsibilities:
The Clinical Nurse Supervisor will be responsible for:
- Coordinating comprehensive patient care by assessing client conditions, advocating for preventive measures, and managing service delivery.
- Regularly evaluating the impact of nursing services on patients and their families.
- Conducting necessary assessments and documentation for home care patients.
- Collaborating with physicians to modify Patient Care Plans as needed.
- Performing thorough assessments of client needs.
- Facilitating patient service coordination.
- Submitting clinical and progress notes consistently to document care provided.
- Participating in case discussions to address patient-related challenges.
- Engaging with other health team members as required.
- Working with external agencies to ensure seamless care continuity.
- Continuously improving nursing competencies through ongoing education and training.
- Adhering to all applicable federal, state, and accreditation standards.
Compensation: $45.00 per hour
