Job Overview

Position Summary:

Under the guidance of the Director of Psychiatric Services, the Mental Health Clinical Coordinator is responsible for orchestrating psychiatric clinical services for specialized care patients. This role includes overseeing the management of psychiatric provider panels, offering both clinical and administrative assistance to these providers, and delivering direct clinical services to patients by performing essential patient care tasks aligned with the responsibilities of a medical assistant.

Key Responsibilities:

  • Act as a resource for the entire psychiatric services team, particularly concerning specialized patient populations served by their provider.
  • Assist with special initiatives developed by the psychiatric services team.
  • Participate in regular team meetings with psychiatric providers to discuss high-risk cases and devise strategies for delivering patient-centered and trauma-informed care.
  • Serve as the primary point of contact and care for patients between visits via a dedicated telephone line.
  • Ensure all scheduling requirements are met for assigned provider(s).
  • Complete and document clinical tasks as requested by the provider, including tracking prior authorizations, sharing lab results, and making medication-related calls to patients or relevant third parties.
  • Monitor follow-up and health maintenance needs for all patients on the assigned providers' panels to ensure proper care for at-risk populations.
  • Manage a database of patients requiring psychiatric consultation to ensure follow-up.
  • Engage patients in health coaching and motivational interviewing.
  • Provide patient advocacy and navigation for Family Health Center services and community resources.
  • Prepare examination rooms with the necessary equipment and supplies for various visits.
  • Direct patients into the examination room promptly after registration to ascertain the reason for their visit.
  • Conduct medication reconciliations.
  • Review patient information based on presentation and discuss relevant medical concerns with nursing staff or the provider.
  • Maintain and prepare note templates based on the purpose of the encounter.
  • Keep a repository of psychiatric clinical questionnaires, registration and intake forms, and consent documents in multiple languages, explaining the purpose of all forms to patients.
  • Document quality control measures and procedures for point-of-care devices.
  • Implement infection control procedures for sterilization and cleaning.
  • Address patient complaints and escalate those requiring further action as appropriate.
  • Participate in root cause analysis as needed.
  • Adhere to all organizational and departmental policies and procedures, including confidentiality, dress code, and attendance policy.
  • Maintain a safe and healthy environment for patients, visitors, and co-workers.
  • Accept responsibility and accountability for work as defined by role and responsibilities.
  • Make decisions based on facts and sound judgment.
  • Take initiative to support the transformation to a patient-centered medical home model.

Qualifications and Education Requirements:

To be considered for this position, candidates must meet one of the following combinations of education and experience:

  • High School Diploma or GED.
  • Three years of experience in primary health care, psychiatry, or a related health care setting.
  • Phlebotomy training is preferred.
  • Certification as a Medical Assistant or Registered Medical Assistant from an accredited training program is required, or a specialty certification and/or a minimum of three years of experience in a specialty.
  • Strong written and verbal communication skills, along with excellent computer proficiency (Microsoft Word, Excel, and Outlook).
