We are seeking a highly skilled Admissions Coordinator to join our team at West Hills Health and Rehab Center. The successful candidate will be responsible for ensuring a smooth and efficient admissions process for our patients.
Key Responsibilities- Comply with facility attendance policies and maintain regular, reliable, and punctual attendance.
- Provide tours of the facility, explain services, and distribute brochures to prospective patients and their families.
- Perform daily rounds to ensure vacant rooms are presentable during tours.
- Determine primary and secondary payers, obtain all pertinent documents, and prepare and type Admission Agreements.
- Verify Medicare coverage for Part A and B, obtain a copy of the resident's Medicare card, and verify available days, prior stays, and secondary payers.
- Verify Medi-Cal eligibility, determine and collect the share of cost, obtain TARs, and Medi-Cal plastic ID cards.
- Notify SSI of admission and assist in the application for long-term care.
- Verify HMO plan coverage, obtain authorization, number of days covered, and copy of the card.
- Work with resident's family or responsible party to obtain signatures on consent forms and assist in state and regulatory application processes.
- Initiate TARs on the day of admission for all patients with Medi-Cal coverage, even if they are admitted under Medicare.
- Educate social service and office staff on admitting functions to maintain admitting flow while the Admitting/Marketing Coordinator is performing external marketing functions.
- Take inquiries and complete forms contained in the resident's financial folder and admission packet.
- Participate in admissions and rehab meetings.
- Assist in calling hospitals for referrals and daily follow-up.
- Coordinate with case managers, discharge planners, and social workers from referral sources to ensure smooth transfers of residents.
- Conduct tours of the facility as needed for discharge planners and families.
- Inform nursing and other department managers of potential admissions.
- Track HMO patients' length of stay and progress.
- Follow up with family members during placement on discharge status.
- Participate in the internal quarterly quality assurance program.
- Perform other duties as assigned by the Administrator.
- Knowledge of long-term care patient needs.
- Fully vaccinated against COVID-19, including booster shots.
- Mental maturity to deal with fast-paced work environments.
- Ability to cope with mental and emotional stress related to working with persons who are ill, disabled, elderly, and/or emotionally upset, and/or memory impaired.
- Ability to manage resident behavior, including verbal outbursts, combative behavior, striking out, and memory impairment.
- Ability to speak Spanish or any other non-English language (helpful but not required).
- Awareness of and understanding of the needs of the residents.
