Job Description

Job Title: Supportive Services Navigator - Spanish Required

Job Summary: We are seeking a highly skilled and compassionate Supportive Services Navigator to join our team at Warren Village Inc. The ideal candidate will possess excellent communication skills, a strong understanding of case management principles, and the ability to work effectively with diverse populations.

Key Responsibilities:

  • Conduct outreach and assessments to identify potential residents' needs and develop individualized support plans.
  • Provide case management services, including resource navigation, coaching, and crisis intervention, to residents and their families.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery and effective resource allocation.
  • Maintain accurate and timely records of resident interactions, progress, and outcomes.
  • Participate in regular team meetings and training sessions to enhance knowledge and skills.

Requirements:

  • Fluency in English and Spanish, with excellent verbal and written communication skills.
  • Associate's degree in Social Work, Human Services, or a related field, or equivalent experience.
  • 2+ years of experience in case management, social services, or a related field.
  • Ability to work effectively in a fast-paced environment, prioritize tasks, and manage multiple responsibilities.
  • Strong analytical and problem-solving skills, with the ability to think critically and creatively.

Preferred Qualifications:

  • Bachelor's degree in Social Work, Human Services, or a related field.
  • Experience working with homeless populations, housing stabilization, and supportive services.
  • Knowledge of trauma-informed care, motivational interviewing, and crisis intervention techniques.

Work Environment: The working environment is typically that of an office using a personal computer, adding machine, and other equipment. The person in this position needs to occasionally move about inside the office/facility to access file cabinets, storage spaces, access units and building spaces by utilizing stairs, office machinery, etc. Constantly operates a computer and other productivity machinery, such as a calculator, copy machine and computer printer. Must be able to exchange accurate information in these situations.
