Position Overview:

We are seeking a dedicated Case Manager to provide essential support to our SMI Adult clientele at our designated location.

This role involves delivering direct crisis intervention services, facilitating support, transportation, and educational resources to families and individuals. Responsibilities include conducting comprehensive family needs assessments, making suitable referrals to collaborative partners, and ensuring a seamless continuum of care for clients. The Case Manager will also play a key role in coordinating communication with various departments and collaborators to enhance service delivery. This is a Full-Time position that requires adaptability to meet the scheduling needs of clients and families.

Key Responsibilities:
  • A bachelor's degree in a behavioral health-related field or compliance with BHT requirements is essential.
  • Prior experience working with Severely Mentally Ill Adults and familiarity with the adult care system is mandatory.
  • Flexibility in work hours and the ability to meet clients in diverse settings, including their homes and community spaces, is required.
  • Possession of a Fingerprint clearance card or eligibility to obtain one is necessary.
  • A valid Arizona driver's license with a clean driving record for the past three years is required.
  • Access to reliable transportation is essential.
  • Bi-lingual candidates (Spanish) are encouraged to apply, with differential pay available.
  • Candidates must be at least 21 years of age at the time of employment.

  • Ability to maintain composure in crisis situations, work autonomously and collaboratively, and demonstrate empathy towards individuals from diverse backgrounds.
  • Adherence to professional conduct aligned with ethical standards and agency policies.
  • Fostering positive relationships with team members, stakeholders, and community partners is crucial.
  • For those driving during work hours, a valid Arizona driver license and appropriate liability insurance are required, along with a clean driving record.
  • All employees must possess at least a high school diploma or equivalent, with age requirements varying based on the role.
