- Collaborate with Interdisciplinary Teams: Work closely with RN Care Managers, healthcare providers, and other stakeholders to ensure comprehensive and coordinated care.
- LPN License: Current LPN license required.
- Experience: Previous experience in care coordination, case management, or a related field preferred.
- Skills: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and strong analytical and problem-solving skills.
