+ Formulates an occupational therapy care strategy with achievable and measurable outcomes in partnership with the child, family, and healthcare provider, ensuring alignment with organizational standards.

+ Educates clients and advocates for their rights and responsibilities to facilitate informed choices regarding their care plan.

+ Designs, executes, and modifies a personalized care plan that aligns with the child's age, condition, and specific requirements.

+ Carries out therapeutic activities aimed at promoting independent living for the child and family.

+ Regularly evaluates the child's needs for occupational therapy interventions and assesses the effectiveness of these interventions through documented reports.

+ Connects clients with relevant internal and external community resources to achieve desired outcomes when necessary.

+ Initiates appropriate diagnostic, preventive, and rehabilitative actions to ensure the child's safety and support the ongoing development and adjustment of the care plan.

+ Monitors and records progress towards established goals, coordinating discharge planning with the child, family, and other healthcare professionals.

Qualifications:

+ A Master's or Doctorate degree in Occupational Therapy from an accredited institution.

+ A minimum of one year of clinical experience in Occupational Therapy is preferred, with experience in home health settings being advantageous.

+ A valid Occupational Therapy License in the State of Maine and a current CPR certification.

+ Must possess a reliable, insured vehicle and a valid Maine driver's license.
