The Licensed Practical Nurse (LPN) plays a crucial role in delivering safe and dependable patient care under the guidance and oversight of a Registered Nurse (RN). The LPN is involved in monitoring, observing, and reporting patient responses to the established care plan.
We are seeking full-time shifts, either during the day or night, operating from 7:00 am to 7:00 pm or 7:00 pm to 7:00 am, totaling 72 hours per pay period.
Our Acute Rehabilitation unit consists of 10 beds and is accredited by CARF (Commission on Accreditation of Rehabilitation Facilities). This unit specializes in stroke and brain injury rehabilitation. Our patient demographic primarily includes individuals with neurological conditions, as well as those recovering from multi-trauma and prolonged hospital stays. We foster a supportive team environment that emphasizes collaboration among therapists and other departments, aiming to help our patients regain independence and return home. The average duration of stay for patients is around two weeks, with staffing comprising two nurses and one technician during the day and two nurses at night.

Key ResponsibilitiesClinical Excellence
- Collaborates with the healthcare team to deliver patient care and achieve optimal outcomes and patient-centered care goals. Seeks guidance and supervision when implementing new or unfamiliar nursing practices.
- Contributes to the admission assessment by accurately collecting, reporting, and documenting both objective and subjective data in a timely manner.
- Assists in identifying suitable patient care interventions to ensure comfort, support physiological functions, and maintain a safe environment.
- May receive and process physician orders in accordance with hospital policy, implementing orders following RN acknowledgment.
- Administers medications and performs tasks within the scope of practice.
- Integrates a multidisciplinary approach to ensure comfort and safety for patients.
- Works with patients and families to anticipate necessary steps for safe transitions in care.
Effective Communication
- Informs the assigned RN of any changes in patient condition promptly.
- Collaborates with the RN and other healthcare team members to communicate progress towards agreed-upon care goals, assisting in updating the care plan as needed.
- Exhibits strong verbal and written communication skills with patients, families, and healthcare team members.
- Ensures real-time documentation in the Electronic Medical Record (EMR) in line with hospital and unit policies.
- Completes hand-offs with meticulous attention to detail, prioritizing safety.
Patient Education
- Incorporates patient and family education throughout the hospitalization, covering the care plan, procedures, and discharge instructions.
- Integrates preventive health maintenance into the educational framework.
- Provides discharge instructions after review and approval by the RN.
- Assesses and selects teaching strategies that are appropriate for the patient's and family's age, culture, readiness to learn, and comprehension level.
- Consults with relevant hospital resources to provide specialized education.
- Demonstrates a commitment to patient and workplace safety by preventing errors and reporting concerns immediately.
Basic Performance Criteria

Education:
Completion of an accredited LPN program and/or eligibility for licensure examination.
Experience:
Prior experience in a rehabilitation setting.
Licenses/Certifications:
· Current, unexpired LPN licensure in the State of Iowa or a compact state.
· Current BLS certification.
· Valid driver's license for work-related driving.
· Completion of IV Therapy certification.
Knowledge/Skills/Abilities:
· Proficient in reading, writing, speaking, and understanding fluent English.
· Demonstrates manual skills and techniques necessary for basic nursing procedures and operation of specialized equipment.
· Ability to complete a basic computer course on the Electronic Medical Record (EMR) through in-hospital training.
Other:
· Familiarity with standard equipment used in the role.
· Age-Specific Requirements: Patients served are 18 years and older.
