Case Manager

Prairieland Service Coordination, Inc. is seeking a highly motivated and experienced Case Manager to join our team. As a Case Manager, you will play a vital role in supporting individuals with developmental disabilities in achieving their goals and living fulfilling lives.
Key Responsibilities- Assess individual needs and develop personalized plans to ensure the provision of necessary services.
- Coordinate with provider agencies, service monitoring, and individual advocacy to ensure the best possible outcomes for our clients.
- Complete the PAS process, following the PAS manual procedures, to ensure accurate and timely service delivery.
- Advocate on behalf of individuals and their families to assist in obtaining services to which they are eligible and have chosen.
- Review individual medical, residential, vocational, educational, legal, and financial status needs and individual choices through Visiting Notes, as required.
- Schedule and complete Discovery and Personal Plan process annually, or as required.
- Maintain individual files as specified by program and agency guidelines.
- Provide problem-solving consultation with provider agencies following the DHS Conflict Resolution guideline.
- Schedule and complete all Visits, Discovery, and Personal Plan process as required.
- Coordinate specialized services for eligible Bogard individuals as needed and submit summary reports to external agencies as appropriate.
- Monitor services provided to Bogard class members on a monthly basis and advocate for any unmet needs.
- Enter billings daily into Birdseye to ensure all time reporting for payroll is submitted on time and accurately.
- Participate in scheduled all staff meetings and mandated training sessions as recommended by the Executive Director and/or Program Manager.
- Alert the Program Manager of PSCI to problematic issues, trends, or service gaps.
- A bachelor's degree in human services required and QIDP eligibility as established by Department of Human Services.
- The ability to organize, plan, and follow a schedule on a daily, weekly, monthly, and annual basis.
- A working knowledge of the developmental disabilities service delivery system, including recipient eligibility requirements and procedures.
- The ability to work and interact effectively with professionals, paraprofessionals, social service organizations, and clients, including their families.
- The flexibility to work hours outside of the normal workweek.
- The ability to follow directions, accept constructive criticism, and accept responsibility for their own actions.
- The ability to maintain a valid Illinois Driver's License and personal auto insurance and maintain a good motor vehicle record.
- The ability to read, write, and communicate effectively.
- The ability to ambulate, sit, and bend.
- The ability to carry files.
- The ability to lift up to 5 lbs. or pull a carrier.
