Job Summary

PATH is seeking a dedicated Case Manager to join our Supportive Housing Services (SHS) team. As a Case Manager, you will work closely with clients to provide intensive case management services, helping them achieve their goals and improve their quality of life.

Key Responsibilities
  • Provide strengths-based case management and service coordination to clients
  • Collaborate with clients to create person-centered plans that address short-term and long-term goals
  • Consult with the team to conduct crisis and risk assessments and provide crisis intervention services
  • Mediate and advocate on behalf of clients to assist with obtaining and keeping stabilizing resources
Requirements
  • Two years of experience in healthcare, case management, homeless services, or a related field
  • Ability to work flexible hours, including evenings and weekends
  • Employment eligibility verification
  • Proof of full COVID-19 vaccination
  • Successful completion of a tuberculosis test, background screening, and drug test
