Case Manager

PATH is seeking a dedicated Case Manager to join our Supportive Housing Services (SHS) team. As a Case Manager, you will work closely with clients to provide intensive case management services, helping them achieve their goals and improve their quality of life.
Key Responsibilities- Provide strengths-based case management and service coordination to clients
- Collaborate with clients to create person-centered plans that address short-term and long-term goals
- Consult with the team to conduct crisis and risk assessments and provide crisis intervention services
- Mediate and advocate on behalf of clients to assist with obtaining and keeping stabilizing resources
- Two years of experience in healthcare, case management, homeless services, or a related field
- Ability to work flexible hours, including evenings and weekends
- Employment eligibility verification
- Proof of full COVID-19 vaccination
- Successful completion of a tuberculosis test, background screening, and drug test
