Role Overview:

The Long-Term Services and Supports (LTSS) Care Coordinator plays a vital role in developing personalized, goal-oriented, and culturally sensitive care plans. This position ensures that individuals receiving LTSS access services in a supportive, effective, and efficient manner. Care coordination encompasses case management, disease management, discharge planning, transition planning, and addressing social determinants of health while facilitating integration into the community.

Key Responsibilities:

  • Evaluate, strategize, and implement individualized care plans tailored to the specific needs of each person, focusing on the least restrictive care options.
  • Identify and facilitate referrals to social service programs, including financial, psychosocial, and community resources.
  • Oversee the person-centered service/support plan throughout the entire care continuum.
  • Communicate essential health-related information to all stakeholders to ensure timely and quality coordinated care.
  • Advocate for individuals and families to ensure their needs and preferences are represented and supported by the health care team.
  • Conduct home visits in collaboration with the individual and care team, which may include community service coordinators.
  • Perform in-person visits to various settings, including nursing homes, assisted living facilities, hospitals, or homes.
  • Act as a resource for community care coordinators when applicable.

Required Qualifications:

  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related field.
  • Minimum of 2 years of experience in a community health setting within a health care role.
  • At least 1 year of experience working with individuals with long-term care needs.
  • Experience with home and community-based waivers in Kansas is preferred.
  • Proficiency in MS Word, Excel, and Outlook.
  • Willingness to travel within the assigned region to visit Medicaid members in various settings.

Preferred Qualifications:

  • Licensed Social Worker or a clinical degree.
  • Experience managing populations with complex medical or behavioral needs.
  • Familiarity with electronic charting systems.
  • Experience in coordinating community resources.
