The Long-Term Services and Supports (LTSS) Coordinator plays a vital role in developing a personalized, goal-oriented, and culturally sensitive plan to ensure individuals receiving LTSS access services in a supportive, effective, efficient, timely, and cost-effective manner. This role encompasses care coordination, case management, disease management, discharge planning, transition planning, and addressing social determinants of health while facilitating integration into the community.
As a team member, you will enjoy the flexibility to work remotely while tackling significant challenges.
Key Responsibilities:
- Evaluate, strategize, and implement tailored care plans that prioritize the individual's needs and direct them towards the most suitable, least restrictive care options.
- Identify and initiate referrals to social service programs, including financial assistance, psychosocial support, and community and state resources.
- Oversee the person-centered service/support plan throughout the entire care continuum.
- Communicate essential health-related information to all stakeholders to ensure that quality coordinated care and services are delivered promptly.
- Advocate for individuals and families to ensure their needs and preferences are fully represented and supported by the health care team.
- Conduct home visits in collaboration with the individual and care team, which may include a community service coordinator.
- Perform in-person visits to various settings, including nursing homes, assisted living facilities, hospitals, or homes.
- Act as a resource for community care coordinators, as applicable.
Your efforts will be recognized and rewarded in an environment that challenges you while providing clear guidance on achieving success in your role, along with opportunities for professional development in other areas of interest.
Required Qualifications:
- Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related field in human services.
- At least 2 years of experience in a community health setting within a health care role.
- A minimum of 1 year of experience working with individuals with long-term care needs.
- At least 1 year of experience with individuals receiving services under home and community-based waivers.
- Proficiency in MS Word, Excel, and Outlook for at least 1 year.
- Willingness to travel within the assigned region to visit Medicaid members in their homes or other locations, including community centers and hospitals.
Preferred Qualifications:
- Licensed Social Worker or relevant clinical degree.
- Experience managing populations with complex medical or behavioral needs.
- Familiarity with electronic charting systems.
- Experience in coordinating community resources.
