Position Overview:
The Long-Term Services and Supports (LTSS) Coordinator plays a crucial role in developing personalized, culturally sensitive, and actionable plans to ensure individuals receiving LTSS have access to services that are supportive, effective, efficient, timely, and cost-effective.
Key Responsibilities:
- Assess, design, and execute individualized care strategies tailored to each person's needs, focusing on the least restrictive level of care.
- Identify and facilitate referrals to social service programs, encompassing financial, psychosocial, community, and state support services.
- Oversee the person-centered service/support plan throughout the entire care continuum.
- Communicate essential health-related information to all stakeholders to guarantee timely and quality coordinated care.
- Advocate for individuals and families to ensure their needs and preferences are fully represented and supported by the health care team.
- Conduct home visits in collaboration with the individual and care team, which may include community service coordinators.
- Perform in-person visits to various settings, including nursing homes, assisted living facilities, hospitals, or homes.
- Serve as a resource for community care coordinators, if applicable.
Qualifications:
- Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related field.
- Minimum of 2 years of experience in a community health setting within a health care role.
- At least 1 year of experience working with individuals with long-term care needs.
- 1 year of experience with individuals receiving services through home and community-based waivers in Kansas.
- Proficiency in MS Word, Excel, and Outlook.
- Willingness to travel within the assigned region to visit Medicaid members in various settings.
Preferred Qualifications:
- Licensed Social Worker or relevant clinical degree.
- Experience managing populations with complex medical or behavioral needs.
- Familiarity with electronic charting systems.
- Knowledge of arranging community resources.
