Job SummaryAs an LPN HCT Evening Care Specialist, you will be responsible for providing direct patient care, adhering to established department guidelines, policies, and procedures. You will work closely with our interdisciplinary team to ensure the best possible outcomes for our residents.
Key Responsibilities- Provide nursing care for assigned patients/residents, including admissions, discharges, and transfers.
- Develop and implement individualized care plans, ensuring the physical and emotional needs of our residents are met.
- Administer medications, maintain accurate records, and communicate effectively with the healthcare team.
- Supervise and guide Certified Nursing Assistants (CNAs) in their daily activities.
- Participate in departmental activities, meetings, and in-services, staying up-to-date on the latest best practices and regulations.
- Current LPN license by the Florida State Board of Nursing.
- Current American Heart Association BLS Certification.
- IV Certification preferred.
- High School Diploma or equivalent.
- Graduate of an accredited school of practical nursing.
