Hospice Nurse Practitioner Per Diem Opportunity

JOB SUMMARY

We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team on a per diem basis. The ideal candidate will have at least 1 year of Hospice experience and a strong background in providing high-quality patient care.

KEY RESPONSIBILITIES
  • Deliver medical care and manage terminal illness in accordance with state regulations and under the supervision of a physician.
  • Conduct thorough patient histories and physical exams, order and interpret lab and diagnostic tests, diagnose conditions, and document findings.
  • Develop and manage individualized care plans, prescribe medications and treatments, and coordinate referrals and hospice services.
  • Perform face-to-face hospice visits to assess eligibility for continued care and provide clinical data for recertification.
  • Collaborate with the interdisciplinary team to create, review, and update patient care plans.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination.
  • Stay informed about Medicare/Medicaid and state/federal regulations for hospice services and ensure adherence to organizational policies.
QUALIFICATIONS
  • Graduation from an accredited nursing school.
  • Current DEA license.
  • Completion of an accredited Nurse Practitioner Program meeting board standards or certification by an equivalent national organization.
  • Current nursing license and CPR certification.
  • Master's degree, with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience.
  • Certification from an approved advanced practice nursing body.
  • Strong observational, verbal, and written communication skills; problem-solving abilities; and nursing competencies.
