Job Summary

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at ABL Health Care, LLC. As a Hospice Nurse Practitioner, you will play a vital role in providing high-quality, patient-centered care to individuals and families in need of end-of-life support.

Key Responsibilities
  • Deliver Medical Care and Manage Terminal Illness: Provide comprehensive medical care to patients, manage their terminal illness, and alleviate symptoms in accordance with state regulations and under the supervision of a physician.
  • Conduct Patient Assessments and Develop Care Plans: Conduct thorough patient histories and physical exams, order and interpret lab and diagnostic tests, diagnose conditions, and document findings. Develop and manage individualized care plans, prescribe medications and treatments, and coordinate referrals and hospice services.
  • Perform Face-to-Face Hospice Visits: Perform face-to-face hospice visits to assess eligibility for continued care, providing clinical data for recertification.
  • Collaborate with the Interdisciplinary Team: Collaborate with the hospice medical director, interdisciplinary team, and other healthcare providers to ensure seamless care coordination and patient outcomes.
  • Stay Informed and Compliant: Stay informed about Medicare/Medicaid and state/federal regulations for hospice services, ensuring adherence to conditions of participation and other healthcare regulations.
Qualifications
  • Graduation from an Accredited Nursing School: Graduation from an accredited nursing school and current DEA license.
  • Advanced Practice Nursing Certification: Completion of an accredited Nurse Practitioner Program meeting board standards or certification by an equivalent national organization, or a registered nurse with an approved program.
  • Master's Degree and Hospice Experience: Master's degree, with at least one year of Nurse Practitioner experience and one year of hospice or palliative care experience.
  • Certification and Licensure: Certification from an approved advanced practice nursing body and current nursing license and CPR certification.
