Job Summary

The Registered Nurse (RN) plays a vital role in our collaborative patient care team, ensuring an exceptional patient experience. Our RNs are empowered to practice and contribute throughout the care experience, providing direct patient care while demonstrating servant leadership and leveraging our interdisciplinary approach to integrated care.

Key Responsibilities
  • Deliver evidence-based nursing care and patient education
  • Perform care management by connecting patients with specialists and following up to ensure patients receive optimal care
  • Utilize population management tools to ensure patients receive care based on industry-wide recommendations
  • Provide phlebotomy services, perform in-house laboratory tests, and work with providers to determine when to send out specialty labs
  • Screen for vaccine-preventable diseases and administer primary care and travel-related immunizations
  • Manage RN schedules and appointments, taking initiative to contact patients when needed and demonstrating knowledge of when to consult with a provider
  • Comply with all policies, procedures, and protocols of the practice, including performing specific care under standing orders
  • Provide general triage services to walk-ins and over the phone, assessing patient needs and directing appropriate care
  • Participate in quality improvement programs to ensure the practice is performing efficiently and in accordance with Crossover's policies
  • Demonstrate proficiency in the use of healthcare analytic tools, systems, and analysis
  • Participate in the implementation of new health technologies, products, services, and programs that establish Crossover Medical Group as a recognized thought leader for employer-based healthcare and lifestyle medicine programs
  • Manage population health programs and clinic outreach initiatives
  • Serve as a resource to other nurses and the care team, participating in the training and onboarding of new nurses
  • Assist in performing Quality and Compliance checks required on a daily, weekly, and monthly basis
  • Contribute to reporting requirements related to incident reporting and compliance
Requirements
  • Graduate from an accredited school of nursing
  • Minimum 3 years of clinical experience
  • Current Registered Nurse license in the state of practice
  • Current BLS (Basic Life Support) certification
  • Preferred Qualifications: Bachelor of Science Degree in Nursing (BSN), outpatient care and urgent care experience, knowledge of Apple and Google-based technology or client technologies such as Microsoft or HP
