Job Title: Integrated Care Coordinator

At St. Vincent de Paul Village, we are seeking a highly motivated and organized Integrated Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in coordinating patient care and ensuring seamless transitions between services.

Key Responsibilities:
  • Provide care management and health care navigation services to patients within our Federally Qualified Health Center (Village Health Center; VHC)
  • Utilize strong customer service skills to perform outreach and engagement in-person and via phone, assessment/evaluation of needs, provides education, meets and/or distributes materials to patients, coordinates and schedules services, and/or performs warm hand-offs to needed services
  • Represent Father Joe's Villages (FJV) to the Senior Oral Health Partnership (SOHP) and attend meetings, coordinate activities, and report to the SOHP in collaboration with the Dental Director
  • Participate in meetings, presentations, workgroups and/or learning collaboratives, as assigned and attend the weekly clinical coordination meeting
  • Facilitate outreach and referrals to other organizations in an attempt to secure services unavailable at Father Joe's Villages Dental Clinic but recommended by the dental providers
  • Search the patient database to identify seniors who may need navigation of services and help to facilitate such
  • Work with health care providers to facilitate services to patients from dental to other VHC departments and vice versa
Requirements:
  • High School Diploma or GED
  • Bachelor's degree preferred in related field
  • Minimum 2 year experience in a healthcare setting
  • Equivalent combination of education and experience that provides the skills, knowledge, and ability to perform the essential job duties
  • Bilingual (Spanish/English) written and verbal, required
  • Completion of a Dental Assistant training program preferred but will train on the job for the right candidate
  • Experience with electronic health record systems
  • Basic user of MS Office, and other essential computer skills
  • Current American Heart Association healthcare provider CPR (BLS)
Preferred Qualifications:
  • Supporting individuals with disabilities, age-related conditions, mental illnesses, or challenging behaviors highly preferred
  • Knowledge of Medi-Cal, Medicare, Managed Care Plans, and insurance guidelines preferred
