We are seeking a skilled Schedule Specialist to join our team at Athens Limestone HomeCare, a part of LHC Group. As a Schedule Specialist, you will play a critical role in managing patient referrals and visit schedules, ensuring seamless care delivery to our patients.

**Key Responsibilities**

  • Utilize an automated scheduling system to maintain a calendar of services for both episodic and per visit customers.
  • Process workflow for requested scheduled, missed, rescheduled, reassigned, declined, and delivered visits.
  • Monitor pending referrals daily and assign licensed professionals and case managers for all start-of-care visits.
  • Communicate daily with field staff regarding any visits in late, pending, or incomplete status for resolution as appropriate.

**Requirements**

  • High school education or equivalent.
  • Minimum one year of scheduling experience in a healthcare setting using an online scheduling system is preferred.
  • Exceptional organizational, customer service, communication, and decision-making skills are required.
  • Working knowledge of state and federal regulations governing OASIS visits, supervisory, and reassessment visits.
