As a Medical Social Work Professional, you will be responsible for providing high-quality medical social work services to patients receiving care in the hospital setting. Your expertise will be utilized to assess the social, psychological, cultural, environmental, and financial situations of each referred patient, as well as their disposition needs.
Key Responsibilities:- Collaborate with patients, families, healthcare team members, and community organizations to identify needs and resources in the hospital and community.
- Develop and evaluate effective transition plans, taking into consideration the client's diagnosis, prognosis, course of treatment, past and present services, short-term and long-term goals, provider options, and available healthcare benefits.
- Function in a hospital setting, utilizing strong advocacy skills to ensure the best possible outcomes for patients.
- Certification in Case Management (CCM, CMAC, or ACM-SW) required within one year of eligibility.
- BLS (Basic Life Support) certification required within 90 days of hire.
- Master's degree in Social Work (MSW) required.
- One year of related experience in a healthcare setting.
