Position Overview:
This role entails overseeing the Enhanced Community Coordination (ECC) team members. The ECC is dedicated to providing comprehensive Service Coordination for individuals with intellectual and developmental disabilities who have transitioned from or been diverted from institutional settings. The ECC Service Coordination offers robust and adaptable support to ensure individuals thrive in community environments, including crisis prevention and management assistance.
Key Responsibilities:
- Supervise ECC staff and ensure compliance with job evaluations and disciplinary processes when necessary.
- Conduct assessments to determine service coordination needs and develop service plans.
- Facilitate referrals and monitor the provision of services, advocating for consumers as needed.
- Provide after-hours crisis support and conduct site visits for monitoring service delivery.
- Engage in service planning meetings and maintain regular contact with consumers, family members, and service providers.
- Prepare and submit reports to relevant authorities as required, including quarterly reports for individuals transitioning from state-supported living centers.
Reporting Structure:
This position reports directly to the COS/CIS/ECC Manager.
Travel Requirements:
Extensive travel within the designated service area is necessary, covering multiple counties. Flexibility in scheduling is essential to accommodate consumer and family needs, including availability for after-hours, weekends, and holidays.
Qualifications:
Minimum:
- Bachelor's degree in a relevant field such as psychology, counseling, or social work from an accredited institution.
- A minimum of two years of experience working with individuals with intellectual and developmental disabilities or dual diagnosis.
- Valid Texas Driver's license and a satisfactory driving record, along with personal automobile liability insurance as mandated by Texas law.
Preferred:
- At least one year of supervisory experience in a related field.
Compensation:
Hourly wage ranges from $22 to $25, depending on qualifications.
