Job Summary

Columbia Community Mental Health is seeking a highly motivated and compassionate Service Coordinator to join our team. As a Service Coordinator, you will play a vital role in empowering individuals with intellectual and developmental disabilities to lead self-determined lives and reach their full potential.

Key Responsibilities
  • Implement and monitor systems that support the IDD community
  • Facilitate health and safety, enhance quality of life, and promote independence, productivity, and social/physical integration
  • Work collaboratively with clients, families, and community resources to develop and implement individualized plans
  • Provide case management services, including assessment, planning, and coordination of services
  • Develop and maintain relationships with community resources and providers to ensure access to necessary services
Requirements
  • Bachelor's degree in a related field, such as social work, psychology, or special education
  • One year of experience in human services, preferably in the IDD field
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
