Key Responsibilities
  • Delivering nursing care in post-acute care settings
  • Adhering to and following care plans for residents
  • Monitoring vital signs such as blood pressure and pulse, providing regular updates to the nursing team
  • Observing residents' conditions continuously and reporting any concerns or emergencies to supervising staff
  • Assisting residents with daily living activities (ADLs), including toileting, grooming, personal hygiene, and feeding
  • Maintaining a clean and sanitary environment by changing linens and sanitizing surfaces
  • Acting as a liaison between residents and healthcare staff
  • Engaging with residents to provide companionship and comfort
  • Communicating with family members and visitors to address their concerns
  • Safely moving residents to and from exam tables, beds, and wheelchairs, and repositioning them as necessary
Qualifications and Skills
  • Current certification as a Patient Care Assistant
  • Basic knowledge of medical terminology and nursing care practices
  • Ability to lift a minimum of 50 lbs
