Job Title: Direct Support Coordinator

Job Summary:

The Direct Support Coordinator will provide habilitative assistance and care to clients living in various Agency programs, including community and supported living arrangements, family support services program, and day/vocational program. Responsibilities include direct daily care, teaching, supervising clients, and maintaining accurate record-keeping.

Responsibilities:

  • Directly supervise clients, ensuring knowledge of their status and location at all times.
  • Assist in developing and implementing Individualized Plans of Service or Individual Educational Plans for clients.
  • Teach, supervise, and assist clients in adult daily living activities.
  • Monitor client progress on goals and collaborate with the team.
  • Protect client rights and report incidents of abuse, neglect, and mistreatment.
  • Maintain accurate record-keeping and implement therapy recommendations.
  • Develop recreation activities for clients and collaborate with relevant parties.
  • Ensure cleanliness and organization of program sites and transportation vehicles.
  • Administer medications, provide routine health care, and first aid.
  • Transport clients to designated activities and programs.
  • Participate in emergency drills and maintain medication certification.
  • Perform other duties as assigned by the Program Director or Assistant.

Qualifications:

  • Completion of Medication Certification Program, PMT, CPR, and First Aid training.
  • Valid driver's license with a satisfactory driving record.
  • High School Diploma or equivalent.
  • Minimum two years of experience working with individuals with developmental and/or behavioral challenges preferred.

Work Environment:

Moderate noise level, potential travel by automobile, and exposure to changing weather conditions.
