Social Services Specialist

We are seeking a highly skilled Social Worker II to join our team at the Texas Department of Aging & Disability Services. As a Social Worker II, you will play a critical role in providing social services to individuals with intellectual and developmental disabilities.
Key Responsibilities- Conduct assessments to identify individual support systems and service needs
- Develop and implement person-centered plans to promote independence and wellness
- Collaborate with interdisciplinary teams to ensure seamless care coordination
- Provide training and education to individuals and families on rights and capacity
- Coordinate admissions, transfers, and discharges
- Master's degree in Social Work or related field
- Knowledge of individuals with intellectual and developmental disabilities
- Ability to communicate effectively in writing and verbally
- Strong analytical and problem-solving skills
