This role involves collecting information to evaluate the individual's support systems and service requirements, participating in interdisciplinary team discussions as necessary.
This position also includes compiling an initial social history and maintaining timely updates.Additionally, this role may involve evaluating the individual's rights and decision-making capacity, providing both individual and group training to encourage the exercise of these rights.
This position aids in coordinating admissions, transfers, transitions, and discharges as required. The Social Worker II operates under moderate supervision, exercising limited initiative and independent judgment.Key Responsibilities:
Regular attendance is essential, with potential requirements for specific shift schedules, rotating shifts, extended hours, and/or overtime in accordance with agency policies, alongside other assigned duties.
May participate in an on-call rotation as needed to support facility operations.Conducts interviews and discussions with individuals, their legally authorized representatives, and involved parties to ascertain essential information and gather background data for assessing the individual's support systems and identifying psychosocial, economic, and physical needs, with updates as necessary.
Interview scenarios include:
upon admission or return from an extended commitment, prior to transitioning to a new residence, during status changes, and in preparation for annual support plan meetings.
Facilitates communication and information sharing among individuals, their representatives, involved parties, and treatment providers.
Oversight ensures that designated staff fulfill liaison roles regarding ongoing care, treatment, and support, which includes maintaining open lines of communication, providing necessary notifications, participating in interdisciplinary meetings as requested, obtaining required consents, and accessing external resources as needed.
Offers guidance to individuals, their representatives, and involved parties on guardianship matters, annual living options discussions, end-of-life planning, and rights in compliance with applicable laws and regulations.
Supervises the coordination and completion of rights and capacity assessments by assigned staff and reviews all documentation.
Delegates tasks to facility staff for admissions, transfers, transitions, discharges, and furloughs to ensure service accessibility through person-centered practices.
Examples include:
conducting facility tours for prospective residents and families, assisting local authorities in obtaining necessary admission documents, and coordinating with receiving facilities for transfers within the system, including transportation and the transfer of resident information and personal belongings.
Coordinates the development and delivery of individual and group training focused on rights, voting, citizenship, and the cultivation of decision-making, person-centered thinking, and self-advocacy skills.
Examples include:
organizing group meetings to inform residents about candidates for political office to facilitate voting, assessing residents' understanding of their rights regarding privacy during visits and calls, and creating educational materials to enhance self-advocacy skills.
Engages in resident and family meetings and presents formal information about facility processes.
Additional responsibilities may include participating in or supporting the agency's disaster response and recovery efforts or Continuity of Operations (COOP) activation, which may necessitate alternate shift patterns or locations.
Required Knowledge, Skills, and Abilities:
Familiarity with individuals with intellectual and developmental disabilities, human development, behavioral and mental health, and the social work standards for case management; knowledge of facility and community resources, as well as the principles and methodologies employed in social work practice.
Strong communication skills, both written and verbal, in individual and group contexts.
Ability to manage assigned property responsibly with limited financial oversight.
Commitment to performing all duties in alignment with the Texas State Board of Social Work Examiners Code of Conduct, the National Association of Social Workers Code of Ethics, and compliance with state and center policies and procedures.
