Hospice Nurse

The Hospice Care Professional provides comprehensive care to patients and their families, focusing on community health education and experience. This role involves building on community resources to plan and direct services that meet individual and family needs within their homes and communities.
Essential Job Functions/Responsibilities
Patient Care
1. Conducts an initial and ongoing comprehensive assessment of patient and family needs, providing a complete physical assessment and history of current and previous illnesses.
2. Provides professional nursing care by utilizing the nursing process, assessing and evaluating patient status, and revising the plan of care as necessary.
3. Initiates the plan of care, makes necessary revisions, and uses health assessment data to determine nursing diagnoses.
4. Develops a care plan that establishes goals based on nursing diagnoses and incorporates palliative nursing actions, involving the patient and family in the planning process.
5. Initiates appropriate preventive and rehabilitative nursing procedures, administers medications and treatments as prescribed, and counsels the patient and family on meeting nursing and related needs.
6. Provides health care instructions to the patient as appropriate, assists with activities of daily living, and facilitates self-sufficiency and optional comfort care.
7. Acts as Case Manager when assigned, coordinating patient care for assigned caseloads.
Communication
1. Maintains accurate and relevant clinical notes regarding patient condition and care given, recording pain/symptom management changes/outcomes as necessary.
2. Communicates with the physician regarding patient needs, reports changes in patient condition, and obtains/receives physicians' orders as required.
3. Coordinates care plans with community health-related persons, ensuring a safe environment for patients.
4. Reports suspected abandonment, abuse, financial exploitation, or neglect of vulnerable adults, submitting reports immediately when reasonable cause is present.
5. Teaches patients and families/caregivers self-care techniques, provides medication and diet instructions, and recognizes opportunities for health counseling.
6. Attends interdisciplinary group meetings, working in cooperation with the family/caregiver and hospice interdisciplinary group to meet emotional needs.
Additional Duties
1. Participates in on-call duties as defined by the on-call policy, ensuring equipment and necessary items/services are available.
2. Supervises ancillary personnel, delegates responsibilities, and assumes responsibility for personal growth and development, maintaining and upgrading professional knowledge and practice skills.
3. Fulfills case assignments, actively participates in quality assessment performance improvement teams, and activities.
Position Qualifications
1. Graduate of an accredited school of nursing, with a registered nurse license in good standing to practice.
2. Minimum one year of experience as a professional nurse within the last three years, or a baccalaureate degree in nursing with a minimum of two years' experience, including one year in public health, home care, or hospice nursing.
3. Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
4. Ability to lift, position, and/or transfer patients, lift supplies and equipment, and possess visual acuity and hearing to perform required nursing skills.
5. Possesses and maintains CPR certification, with a licensed driver and an automobile insured in accordance with state/or organization requirements and in good working order.
