Hospice Community Specialist

Responsible for developing and executing territory and account plans to maintain and grow hospice service admissions from referral partners within the Branch service area. This includes working with community-based referral partners, such as physician offices, senior living facilities, rehab facilities, skilled nursing facilities, and hospitals, to increase referrals and admissions. The ideal candidate will have a strong understanding of the Atlanta healthcare market and experience in sales, business development, and marketing.
Key Responsibilities:
- Develop and execute territory and account plans to increase referrals and admissions
- Work with referral partners to identify opportunities and develop strategies to increase referrals
- Collaborate with the Hospice Atlanta nursing team and facility staff to ensure seamless care delivery
- Provide education and support to patients and families on hospice care and services
- Utilize data and analytics to track progress and identify areas for improvement
Requirements:
- Bachelor's degree in Nursing, Allied Health discipline, Health Administration, or Business Administration
- Two years of successful experience in a healthcare sales environment with a proven track record of success
- Knowledge of the Atlanta healthcare market and experience in hospice and/or home health work
- Strong sales, business development, and marketing skills
- Ability to work independently and be self-motivated
