Job Title: Bilingual Registered Nurse - Home Health Care - Full-Time

Mass General Brigham is seeking a highly skilled Registered Nurse (RN) to join our Ambulatory Care team in Somerville, Massachusetts. As a Bilingual RN, you will play a vital role in providing exceptional patient care to our diverse patient population.

Job Summary:

The successful candidate will work in a dynamic and fast-paced environment, providing comprehensive nursing care to patients in a home health care setting. This is a full-time position, requiring a strong commitment to delivering high-quality patient care and excellent communication skills.

Responsibilities:
  • Provide direct patient care, including assessment, diagnosis, and treatment, in a home health care setting.
  • Develop and implement individualized care plans, taking into account patient needs and preferences.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Utilize advanced communication skills to effectively communicate with patients, families, and healthcare teams.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Master's (or) DNP (or) PhD Degree in Nursing required.
  • Minimum of 5 years direct Family or Pediatric Primary Care experience as an NP required.
  • Fluency in Spanish required.
  • Experience with telehealth / virtual visits preferred.
  • Experience with process improvement / change management preferred.
  • Strong computer skills required.
  • Excellent clear verbal communication skills and proficient writing skills required.
  • Experience with Electronic Medical Records (EMR) and EPIC preferred.
