Central District Health (CDH) is seeking a highly skilled Licensed Clinical Social Worker to join our Family & Clinic Services team. As a key member of our public health team, you will play a vital role in promoting and protecting the health of our communities.

Job Summary:

The successful candidate will be responsible for providing mental health services to individuals and families in our communities. This includes assessing patient needs, developing treatment plans, and providing counseling and support to help patients achieve their goals.

Key Responsibilities:
  • Perform assessments, diagnosis, treatment, and prevention of mental illness, emotional, and other behavioral disturbances;
  • Collaborate with Family and Clinic Service providers on medication treatment;
  • Create treatment plans;
  • Help patients adjust to changes and challenges in their lives;
  • Monitor patient progress throughout their counselling plan;
  • Provide population-based education, prevention, and early intervention with patients and participates in chronic disease management, including psychosocial assessments;
  • Document patient counselling appointments in the medical record.
Requirements:
  • A Master's Degree (or higher) in a direct clinical practice Human Services field is required for this position;
  • Preferred qualifications include experience providing services to children under 12 years of age, the ability to speak, write and read both English and Spanish, and CANS certification.
