Job Summary

We are seeking a dedicated and compassionate Social Worker to join our Hospice Homecare team. As a key member of our interdisciplinary team, you will provide high-quality psychosocial support to patients and their families, promoting their physical, emotional, and spiritual well-being.

Key Responsibilities
  • Conduct thorough psychosocial assessments of patients and families to identify their unique needs and develop personalized care plans.
  • Provide individual and family counseling to address mental and emotional health concerns, reducing barriers to achieving care goals.
  • Identify and address special needs related to cultural and religious diversity, ensuring inclusive and respectful care.
  • Evaluate and address financial and environmental concerns, connecting patients and families with community resources and support.
  • Provide crisis intervention support and psychosocial updates to facility staff and caregivers.
Requirements
  • Master's degree in Social Work from a Council on Social Work Education-accredited program.
  • Active licensure as a Social Worker in the state of employment.
  • Minimum one year of experience in healthcare, with hospice or homecare experience preferred.
  • Reliable transportation, valid driver's license, and car insurance.
