Job Title: Licensed Clinical Social Worker

Central District Health (CDH) is seeking a highly skilled Licensed Clinical Social Worker to join our Family & Clinic Services team. As a key member of our team, you will play a vital role in promoting and protecting health in our communities.

Job Summary:

The successful candidate will be responsible for performing assessments, diagnosis, treatment, and prevention of mental illness, emotional, and other behavioral disturbances. You will assess patient needs, situations, strengths, and support networks to determine their goals and collaborate with Family and Clinic Service providers on medication treatment. Additionally, you will create treatment plans, help patients adjust to changes and challenges in their lives, monitor patient progress throughout their counselling plan, and provide population-based education, prevention, and early intervention with patients and participates in chronic disease management, including psychosocial assessments.

Responsibilities:
  • Perform assessments, diagnosis, treatment, and prevention of mental illness, emotional, and other behavioral disturbances
  • Assess patient needs, situations, strengths, and support networks to determine their goals
  • Collaborate with Family and Clinic Service providers on medication treatment
  • Create treatment plans
  • Help patients adjust to changes and challenges in their lives
  • Monitor patient progress throughout their counselling plan
  • Provide population-based education, prevention, and early intervention with patients and participates in chronic disease management, including psychosocial assessments
Requirements:
  • A Master's Degree (or higher) in a direct clinical practice Human Services field is required for this position
  • Preferred Qualifications: Experience providing services to children under 12 years of age, ability to speak, write and read both English and Spanish, CANS certified (or the willingness to become certified)
