Key Responsibilities:
The selected candidate will primarily operate within the Emergency Department and may also engage with inpatient units, collaborating closely with nursing staff and physicians to assist in the treatment and follow-up of various screening tests. The department includes mental health and substance use advocates, with whom the employee may consult to ensure comprehensive care for patients.
We are in search of a dedicated and empathetic Patient Care Navigator to join our Emergency Department team, focusing on delivering support and assistance to patients dealing with HIV, HCV, Syphilis, Chlamydia, or Gonorrhea. The ideal candidate will be instrumental in ensuring that patients receive thorough care and guidance throughout their experience in the emergency department, facilitating their navigation through the healthcare system.
Qualifications:
- Familiarity with care protocols and guidelines related to HIV, HCV, Syphilis, Chlamydia, and Gonorrhea is preferred.
- Exceptional interpersonal and communication skills are required.
- An empathetic, patient-centered approach to care coordination is essential.
- Strong organizational and time-management skills are necessary.
- Experience with healthcare software and electronic health records (EHR) systems is required.
- Cultural competence and the ability to engage with diverse patient populations are mandatory.
- Comfort in working with patients who may have infectious diseases is required.
- Ability to analyze complex matters and take appropriate action is essential.
- Proficiency in responding to requests independently, whether in writing, in person, or over the phone, is required.
- Accurate typing, data entry, and organizational skills are necessary.
- Capability to adapt to a dynamic environment with shifting priorities and frequent interruptions is required.
- Ability to set priorities effectively and work autonomously is essential.
- Creative problem-solving skills related to technology are required.
Required Qualifications:
- Bachelor's degree in Nursing, Social Work, Public Health, or a related field.
- Current state licensure or certification, where applicable.
Preferred Qualifications:
- A master's degree.
- 1-3 years of prior experience in an emergency department, infectious disease care, or a similar healthcare environment.
