Key Responsibilities- Conduct thorough psycho/social assessments of residents and their family members to identify their needs and develop personalized care plans.
- Coordinate and monitor available services to ensure residents receive the best possible care and support.
- Provide training on social service needs for residents and their families to empower them with knowledge and resources.
- Offer direct casework to residents and families to address their unique challenges and concerns.
- Supervise social workers in the building to ensure they are providing high-quality care and support.
- 2 years of supervisory experience in a social work or geriatric care setting.
- Experience working with the geriatric population and understanding their unique needs and challenges.
- Certified Social Worker in NY State (CSW) or equivalent certification.
- Masters in social work from an accredited program.
- A competitive salary of $90,000 per year.
