Job Summary

As a Care Coordinator with Consumer Direct Care Network, you will play a vital role in supporting individuals with disabilities, chronic health conditions, and the elderly in achieving their long-term care goals. This is a dynamic and rewarding position that requires strong interpersonal skills, attention to detail, and the ability to work independently.

Key Responsibilities
  • Client Support and Advocacy
    • Assist clients in planning, securing, and directing self-directed supports to meet their unique needs and preferences.
    • Provide conflict resolution, interpersonal skills, and cultural sensitivity to ensure a positive client experience.
  • Service Coordination
    • Recruit, screen, and interview potential service providers to ensure the best possible outcomes for clients.
    • Recognize and report instances of abuse and neglect, ensuring the safety and well-being of clients.
  • Client Assessment and Planning
    • Conduct thorough assessments to identify client strengths, capacities, preferences, needs, and desired outcomes.
    • Develop and implement personalized care plans that meet client goals and objectives.
  • Community Resource Connection
    • Assist clients in finding and connecting with community resources, services, and support networks.
    • Provide assistance in planning or coordinating services for life transitions, employment opportunities, and other areas of need.
  • Documentation and Reporting
    • Maintain accurate and timely documentation in the Customer Relationship Management (CRM) database.
    • Compile information for internal or external auditors, ensuring compliance with regulatory requirements.
Qualifications
  • Education and Experience
    • Bachelor's degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing, Sociology, or a related field.
    • 6 years of experience serving individuals with disabilities, chronic health conditions, or the elderly.
  • Skills and Abilities
    • Strong interpersonal and communication skills.
    • Ability to work independently and as part of a team.
    • Comfortable working with clients who may exhibit challenging behaviors.
    • Ability to manage verbally or physically aggressive situations without injury to clients and staff.
Working Conditions

The incumbent typically works in an office environment and uses a computer, telephone, and other office equipment as needed to perform duties. The noise level in the work environment is typical of that of an office. Incumbent may encounter frequent interruptions throughout the workday.
