About the Role

We are seeking a skilled Occupational Therapist to join our team at Mercy Medical Center Home Health and Hospice. As an Occupational Therapist, you will play a vital role in helping patients recover from illness or injury and regain their independence.

Key Responsibilities
  • Provide comprehensive skilled services as ordered by the attending physician, including evaluation, treatment, and education of patients and their families.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure seamless care and optimal patient outcomes.
  • Conduct thorough assessments of patients' physical and cognitive abilities, identifying areas for improvement and developing strategies to address them.
  • Design and implement home exercise programs to improve range of motion, muscle strength, and coordination.
  • Teach patients and their families how to use assistive devices, such as wheelchairs, walkers, and canes, to promote independence and safety.
  • Communicate effectively with patients, families, and healthcare teams to ensure that care is coordinated and patient needs are met.
  • Document patient progress and outcomes in accordance with agency policies and regulatory requirements.
Requirements
  • Current licensure as an Occupational Therapist in the state of employment.
  • Minimum one year of experience as an Occupational Therapist in an acute care setting, with preference given to those with home care experience.
  • Valid driver's license and auto insurance.
  • Ability to meet the physical demands of the job, including lifting, bending, and standing for extended periods.
