Job Title: Transition Services Specialist

At The Pathfinder Network, we are seeking a highly motivated and compassionate Transition Services Specialist to join our team. As a key member of our staff, you will play a vital role in providing transition and reentry services to adults preparing for and releasing from custody to the community.

Key Responsibilities:

  • Conduct assessments and coordinate care for addiction recovery and other resources in the community
  • Prioritize treatment referrals and placement for those assessed in need
  • Maintain knowledge of community resources and collaborate with internal and external partners to support individuals receiving services
  • Develop transition plans with those in services and provide individual sessions utilizing structured models of support
  • Maintain participant records and program documentation according to set standards

Requirements:

  • A high school diploma or equivalent is required
  • One year of related experience
  • Minimum of CADC credential and maintenance of credential required
  • Group facilitation skills and excellent written and verbal communication skills
  • Ability to present and work with diverse audiences, specifically racial, ethnic, gender expression, socioeconomic, education, spiritual, and other cultural backgrounds
