Nurse Case Manager

CVS Health is a leader in innovative healthcare solutions, and we're seeking a dedicated and compassionate Nurse Case Manager to join our team. As a key member of our Integrated Care Management (ICM) team, you will play a vital role in delivering exceptional patient care and outcomes.
About the RoleAs a Nurse Case Manager, you will be responsible for assessing, planning, implementing, and coordinating care for our members who are enrolled in Medicare and Medicaid. You will work closely with members, providers, and community organizations to address the full continuum of their health care and social determinant needs.
Key Responsibilities- Conduct thorough assessments of members' needs and benefits to develop proactive plans of care.
- Apply clinical judgment to incorporate strategies designed to reduce risk factors and barriers, addressing complex health and social indicators that impact care planning.
- Collaborate with supervisors and other key stakeholders to overcome barriers in meeting goals and objectives, presenting cases at interdisciplinary case conferences.
- Utilize case management processes in compliance with regulatory and company policies, ensuring maximum member engagement and discerning their health status and health needs based on key questions and conversation.
- Minimum 3+ years of clinical practice experience.
- Active and unrestricted RN licensure in the state of NJ.
- Willing and able to travel 25-50% of their time using their own vehicle to meet members face-to-face in their assigned area.
- Reliable transportation required, with mileage reimbursed per company expense reimbursement policy.
- Certified Case Manager.
- Minimum 2+ years Care Management, Discharge Planning, and/or Home Health Care Coordination experience.
- Confidence working at home/independent thinker, using tools to collaborate and connect with teams virtually.
