Job Summary

The University of New Mexico seeks a skilled Registered Nurse to serve as a Care Coordinator, responsible for coordinating patient care and facilitating a multidisciplinary approach to achieve quality patient outcomes.

Key Responsibilities
  • Coordinate patient care and facilitate group therapy and/or education sessions
  • Participate in continuous quality improvement activities
  • Develop patient-centered goals in collaboration with physicians, staff RNs, and other healthcare team members
  • Conduct timely discharge planning and ensure continuity of patient care throughout all healthcare settings
  • Collaborate with other departments/services and community healthcare agencies to support quality care in the area of clinical expertise
Requirements
  • 2 years of directly related experience
  • Bachelor's Degree in Nursing (preferred)
  • Active RN license
  • CPR for Healthcare/BLS certification
Working Conditions

The role may require light work, exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. The working conditions may include minor hazards such as physical risks, noise, and exposure to dust and fumes.
