Job Summary

We are seeking a skilled Registered Nurse to join our team at the University of New Mexico. As a Nurse Care Coordinator, you will play a critical role in ensuring high-quality patient care and outcomes.

Key Responsibilities
  • Coordinate patient care services and ensure continuity of care throughout the healthcare system.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Assess patient needs and provide education to patients and families on health status, treatment options, and resources.
  • Collaborate with healthcare team members to ensure effective communication and coordination of care.
  • Participate in quality improvement activities to evaluate patient care systems and implement resolution strategies.
Requirements
  • 2 years of directly related experience as a Registered Nurse.
  • RN licensure in New Mexico or reciprocal agreement.
  • CPR for Healthcare/BLS certification.
Working Conditions

This is a light work position that requires sitting most of the time, with occasional walking or standing. The work environment is minor hazard, with exposure to physical risks, dirt, dust, fumes, and noise.
