We are seeking a skilled LPN to join our Cardio Pulmonary Unit team at Sunnyview Rehabilitation Hospital. As a valued member of our team, you will provide high-quality patient care and support to our patients and families.
Responsibilities- Provide direct patient care and support services to patients and families
- Work collaboratively with the RN to achieve patient goals and outcomes
- Participate in interdisciplinary team meetings to discuss patient care and progress
- Develop and implement individualized patient care plans
- Monitor and report patient progress and concerns to the RN and other healthcare team members
- Current NYS LPN license or limited permit
- BLS certification
- Strong interpersonal skills and ability to work effectively with patients, families, and healthcare team members
