Job Summary

Horizon Home Care & Hospice is seeking a compassionate and dedicated Home Health Aide to provide high-quality patient care in the comfort of their own homes. As a Home Health Aide, you will work under the supervision of a Registered Nurse or Therapist to provide direct assistance with activities of personal care, maintenance of patient health, and facilitation of treatment plans.

Key Responsibilities
  • Provide personal care assistance, including bathing, dressing, transfers, and toileting, under the supervision of a Registered Nurse or Therapist.
  • Assist with daily living activities, such as meal preparation, laundry, and house cleaning, as outlined in the patient's care plan.
  • Explain care procedures to patients and involve them in their care, promoting independence and dignity.
  • Complete mobile care upon arrival and departure from patients' homes, accurately documenting tasks completed in accordance with the care plan.
  • Work cooperatively with patients, care providers, and family members, as well as with Agency staff, to ensure seamless care delivery.
